Documentary Requirements:

1. Latest W2 (form 2316)
2. Latest 1 month payslip
3. Certificate of Employment & Compensation (COE) not older than 3
mos. from date of issue
4. Clear copy of 2 valid IDs plus 3 specimen signatures
5. Latest residential utility bill under your name
SELF-EMPLOYED (BUSINESS) – 1.4% interest per mo.
1. Latest 2 years ITR with audited financial statements
2. Latest 6 months bank statements
3. Latest 3 mos credit card bill statements
4. Clear copy of 2 valid ids plus 3 specimen signatures
5. Updated Business / Mayor’s permit
6. Business Registration
7. Latest Residential utility bill
